If you would like to complete your paperwork at the office, please
arrive about twenty minutes before your scheduled appointment, and:

• Please bring your insurance cards (it would facilitate the check-in process if you could fax a copy of your card/s to us before your appointment. Our fax number is 910-814-1119).
• Please remember that co-payment is expected at the time of service.
• As part of a full eye exam, your eyes will be dilated; please remember to bring a driver if needed.
• Please bring a list of your current medicines.
• Please bring your glasses and/or contacts with you.
• Please give us at least 24 hours notice if you will not be able to keep your appointment.
